This almost a decade old. It was a simple collusion scheme. Slot attendant filled out a handpay slip and obtained supervisor's signature, then cashier gave money to slot attendant so customer could be paid. Actually there was no customer and no jackpot had been hit. No one was paying attention to so many jackpots taking place and often taking place with those same two signatures on the slips that wound up in the cashier's drawer. No auditing function at all.
