While I have not strayed too far off the beaten path, I've explored a little bit and I'd call it a toss up between The Western and Railroad Pass.
Western, Terribles, ... probably a few other candidates depending on weighting factors. The Western had it's rooms shut down by the health department. It's a place for the bums to go. I can't walk in the place since the air is so foul.
To be fair you should pose the question from the 35 casinos that made over $72 million apiece in gaming (Vegas urban area only). If not, then I nominate LARRY'S HIDEAWAY which is also a grade z strip club.

"The place is CRAWLING with roaches!! Its nasty, the staff sucks (except the security) There are bed bugs infesting 2 of the buildings, there is constantly drama going on, and always crime 24hrs a day, and they only have 1 guard who cant keep up with it."

My family member also is highly suspicious that the front office may be involved in this ..."

Of your list of 35 casinos, I've stayed at 7
of them over the years.
Once we found Tuscany Suites, we (I know some
people find it too quiet / boring) have mostly
stayed there.
But we have also been getting some good
room offers from Harrah's properties
lately, so we were at RIO and TI recently.
I don't know if the casino draws any of the trannies or tweakers. And it does lock its doors at dusk. Perhaps trannies simply don't gamble? Not familiar with grade z, but something tells me the average stripper there is as large as a defensive end for the Broncos, and not quite as pretty. Or possibly doesn't have the same plumbing as most women. Or even the same number of chromosomes.
I disrecommend them for other reaons. I lost a $140 slot ticket there and they bascially told me to blow off. Boyd has a lot of variety in its casinos. Compare craps at MSS, Cal and Fremont. Respectively they offer 20X, 3,4,5X and 2X (and I've still played craps at Fremont; that $3 table is a powerful lure). Pretty good VP can be found at all three, with MSS having more good machines. Still, I think the best is at the Cal, a bank of 10/7 DB in $1 and $0.50 with a 1,000 coin bonus on a natural royal in hearts.
Only redeeming factor is it's got 50 cent FPDW. Even then I rather drive down the road and play $1 craps at Joker Wild with 10x odds.

Its probably the safest casino despite having the second worst slots payout in town. And it has some of the oldest hookers in town! Its the top spot for craps dealers and BJ dealers too probably.

Larry's Hideaway is a country western dive bar near the North Las vegas airport, it is not a strip club. You are thinking of Larry's Villa, that is the strip club and it is nasty.
BTW, Larry's Hideaway closed down recently.
As to Siegal Suites, they fax all ids to the cops, so if you have unpaid fines or warrants for anything at all... you pay for your room and an hour later half the swat team is there to take you away but leave your luggage and pets to fend for themselves.
