My vote would go the first place I ever stayed at; The Aladin. It was my first time and I wanted someplace cheap. I was exactly 21 years old and in retrospect I I got exactly what I paid for. The walls were thin, the carpet old and dirty, and not a decent resteraunt in the whole place. At least it was far enough in the past so that they hadn't begun to mess with 6/5 BJ.
I think the worst room I've had was at the Luxor in one of the old Pyramid rooms. Just old and dated and depressing. But I've stayed there several times in the Tower rooms and those are perfectly fine.
Oooh now that I'm thinking about it, once I was staying at the MGM Grand and as I was exiting my room one day there was a bucket full of sh*t right in front of my door. In the room next to mine, apparently sewage had come up through the drains in the sink and bathtub and through the toilet and flooded the room! I dodged a bullet on that one as my room had no issue. But the people in the room next to me had a lot of their stuff ruined by sewage.
They moved me to a new room to get me away from the noisy cleanup crew (and buckets of sh*t in the hallway) and waived the resort fees. I still wonder what the MGM did to make it up to the people in the sewage room...
Outside of Las Vegas at Stateline would be the worst overall that I have stayed at. Whiskey Pete's.
The worst hotel would have to be a tie between hooters hotel in 2007 and Binions in 2007 before they closed down the hotel. I would have rather slept on the floor of a motel 6 considering how badly in shape those two rooms were.

Rancherito or whatever its name was; first craps game ever. Crew no help whatsoever. Crew complained when I didn't tip them. Base dealer said as I left: he could really have hurt us if he knew what he was doing.
Casino that had security frisk me. The old Fitzgerald's I think.
The place that asked me if I wanted a security escort to my car when I cashed my last white chip there.

When I stayed there in the 80's it was a dump with missing rails on the second story walkways, and dirt filled swimming pools.

Yes, I am. They comped me a limo and a suite. From the reviews that I've read the Park MGM has no real middle ground. The reviewers either love it or hate it. The complaints are almost in one of two categories; housekeeping and the whole automated check-in process. I'll bypass the latter with the VIP check-in and judge the housekeeping myself.

If you're limiting it to the casino, probably TI when I first went to Vegas in 2004 - there wasn't a single game really worth playing; the best quarter Jacks or Better was 7/5.

As for me worst hotel? Mehhh...I guess the old IP, but our rooms looked clean at least. But one time, it began to rain in Vegas, we weren't the top floor, but nonetheless the ceiling leaked enough where it looked like the room the ceiling plaster was going to cave in!
We moved out before seeing it happen. We got a creepy Luv Tub room after that. Lol

Worst in Stateline was the Horizon, now the Hard Rock. Which leaves the Montblue (formally Caesars Tahoe) as the current worse, though Harvey’s is a close second.

Park MGM is perfectly fine. I don’t get the bad reviews. People just don’t like change, I guess.
Stayed there 4 nights comped in February. Had zero problems.

I stayed in The luv tub room during CES in 2014, it was the only room my host could comp me on no notice. Weird room.

From Wikipedia: It was previously known by the name of State Line, but was renamed in 1996 to avoid confusion with Stateline in northern Nevada. It is named after the original developer of the town, Ernest Jay Primm.
So the unfortunate stay at Whiskey Pete's could have taken place in State Line.
Then there was the night I was waiting for the parking garage elevator and when it arrived, there were four white girls and four black girls, all in their 20s, looking good too, and they were arguing about something. They stepped out of the elevator and got to throwing punches. And that elevator was on the back side, there was no Security, so it was a pretty good fight, no one interrupted them.
However, there are a ton of casinos in Vegas I've never set foot in; I wouldn't be surprised if there's something worse.

I used to avoid Downtown and stay on The Strip. I had to change my ways because after two knee surgeries I couldn't handle even walking from one casino to the next. Also, Downtown has made some changes for the better while The Strip has gone the other way.
For one thing, when I was on The Strip there was no such thing as 6/5 but you don't have to deal with it downtown. 10X and 20X can be found on many properties as well. The slots-frankly, who cares since I don't play them. There are some weirdos and beggars outside to be sure but they're on the strip too.

There's a free monorail that connects Park MGM to the Bellagio property. It doesn't exactly drop you in the middle of the casino, but it's free. The monorail also stops near Aria too. It's still a pretty long stretch to navigate with a couple of bad knees, but it is free, and better than trying to hoof it all the way.
