If the count is +4
with a 5 upcard, with a 6 upcard

If the count is +5
with a 5 upcard, with a 6 upcard

I know that it's something like 43% chance of bust with a 5, 42% with a 6, but that's at a 0 count.

I'm guessing the chances of a bust are higher with a 6 then a 5 once the count reaches a certain #. (since you're supposed to split 10s at +4 vs a 6, and at +5 vs a 5)

What count are you using? How many decks are there? Does the dealer hit or stay on soft 17?

Correct me if I'm wrong, but I don't think the # of decks matters enough since we're talking about true count here (although the dealer is more likely to draw multiple small cards to not bust the more decks there are, but I don't think that effect is enough to change how you play it).

oh, another thing I noticed..

It seems like 3 of the Illustrious 18 plays are redundant?

16 vs 10
15 vs 10
16 vs 9

Aren't these surrender situations?

Those indexes are included for games that don't offer surrender, and even for those games that do you'd still need them for 3+ card hands (which you normally can't surrender).
