I think it's because the smoker just replaces smoking with eating...and keep in mind how many cigarettes a day a smoker normally smokes. The replacement calories add up.
How about a new policy; nobody is allowed to "officially" attempt the challenge until they can do at least 50.
It's relevant because the stomach has no place to expand into. He might actually be obese or close. You might not think so, and I'm worse off than him a bit, but it's easy being obese. I wouldn't use BMI for anything. It's a completely discredited measure that doesn't take things like muscle mass into account.

I agree - according to BMI, 6'0", 220 is considered low-end obese.
