Does anyone have a list of casinos in Las Vegas that offer this table game? I've searched the internet and cannot find a definitive list.

There isn't a definitive list. It changes month-to-month. I do know it's offered in all Caesar's properties, Venetian/Palazzo, most if not all MGM properties, and from what I've seen nearly all tourist-oriented other casinos. It's pretty popular and continues to grow. I don't think it's offered at the D but I could be wrong. I have seen it at 4Queens and Golden Nugget downtown.
If the House lost every hand, they wouldn't deal the game.

I have played it at the D but not recently so I cannot confirm.
The wiz did a survey in January which is here:

However, it definitely has some errors. As babs says the list changes all the time. For example I have played at Caesars Palace at a $5 minimum. Sometimes the UTH game is part of a carnival games pit that is only open at busier times.

It's at the Wynn, usually 2 or 3 tables at a time. Also Bellagio.
A falling knife has no handle.

I've played it at many of the Strip casinos. Wynn/Encore, Bellagio, Cosmopolitan, Aria, MGM, Venetian/Palazzo(w progressive), Hard Rock, Caesars+. Pretty expensive minimums at some- $15-25 for nights/weekends.

I should *really* do that more with UTH. At those minimums the variance can be brutal and laying out a 4x bet is daunting.

Yeah, it easy to see why the minimums get elevated. The game when played nearly correctly with a 4.15 unit average bet is 0.6% per unit. Perhaps the clientele "support" the red-chip minimums by not playiing well.
Some people need to reimagine their thinking.
