Are these guys for real? If I dont ask for this statement IN PERSON the request I make with the provided forum must be NOTARIZED?
Can I write off that $25 expense too and my time and gas to go get that done

Call them back. Give them your phone number of record and have them call you. When that doesn't work keep asking for the highest level supervisor until someone finally comes to their senses.

Why even bother with those statements. If you keep your own log that's all you'll ever need, and you know it's balls-on accurate.
