What is the difference between Design Patent and Utility Patent for table games? I recall seeing somewhere that "Texas Holdem Bonus" (not UTH) has a design patent, not a utility patent.

The utility patent is going to cover the functionality of the table game. The design will simply cover the look and appearance. From my understanding, the utility patent is going to be more critical since it covers the essential components and characteristics of the game. A design patent may just cover how you have the table layout arranged. I wouldn't take this answer as all-encompassing, but the provisional patent I filed for a table game was for a utility patent only to make sure the functionality of the game was truly unique.
