I feel like I must be missing something simple, but I have a question regarding #s 21, 24, 32.
These deal with 4 to an inside straight, and and specifically with 3 high cards, 2 high cards, and 0 high cards.
What about with 1 high card? Does that fall into a different category that I'm simply missing?

Unlike the other plays you have listed, going for a inside straight with one high card is NEVER the superior play, so it's not listed for conciseness. You will always just hold the high card instead.

I have never been able to understand why in DDB 4 to inside is a play and in JOB it is not? the straight pays the same in both.

So on a total reflop in DDB the two pair is weak, vs JOB. Therefor going for this straight is better. Got it.
