We assume ignorance of the rule if it's a brand-new member who makes duplicate accounts. So, we will usually ban only one account in that situation (given that there's nothing else that was objectionable about the creation of a sock puppet.)
If it's a person with a significant history with the forum, we ban all of their accounts.
Duplicate accounts are often created when someone has been permanently banned and they are trying to sneak back in. Obviously the duplicate account gets zapped.
Blocking a specific IP address is usually not done. But there are exceptions. I think that saying any more would be unwise.

Pretty much as gordonm888 said. My general guideline is to ban the duplicate account for being a duplicate, and the original account gets banned for a different rule violation, as appropriate.
It is not usual practice to document a nuke of a new account on the Wall of Shame. Sometimes these are for spam, sometimes these are for being duplicates. I assume that someone deliberately violating Forum Rules on their first post has no shame, and such humiliation would not benefit the forum.

So what if... someone gets suspended for a routine thing, cursing or something, and a forum enemy creates what looks like a blatant duplicate account for him and posts with it, for the purpose of getting his enemy permanently banned?
The person who just did a minor thing gets banned, and the person psychopathic enough to even think of such a thing rem... wait, never mind!

Your adversary's antisocial impersonation of your mannerism likely appears different enough than your account to preclude a misidentification.
Hypothetically, of course.
If you're on DT, or have a similar out-of-band method to contact a moderator to explain your predicament, that may help.
I also know that creating a clearly duplicate account to bellyache about getting suspended unfairly seldom goes well. Suspensions are rarely more than 7 days, so take the week off, and then state your appeal.

Yes that happened recently and the person was nuked. I disagreed that was the right answer, but Mike’s house Mike’s rules.
