vendor licensing cost?

Hey all, if anyone has any info for me i would really appreciate it. I am trying to gather some info regarding how much it costs to get licensed as a vendor in all of the jurisdictions... Also whether the fee is an annual fee vs. a one time payment etc... Also, any information regarding the cost of getting game approved would be appreciated as well!
