A Rock Gaming casino under construction in Cincinnati collapsed while the construction crew was pouring a floor on the second story. Fortunately, no one died.

I saw that on the news... this is terrible for Horseshoe and ROC. I guarantee you that they are doing their best to minimize what happened, as Cleveland is set to open up in the next few months.
In fact, their website via Horseshoe doesn't even have a press release up about it. Granted, Horseshoe is just the managing partner, but you'd think that people would want to know what happened.

someone is going to get fired for that. If they deviated from design the site engineer is to blame, if not the design engineer is in big trouble. any more links?

A parking garage construction incident with no casualties or injuries? Buildings under construction are held up by a fraction of their design support, a certain failure rate is anticipated. Maybe it got news coverage because it's related to a casino, IDK.
The same day, December 16, 2011, there was a number of other constriction incidents, some with injuries and possibly fatalities.

Really? Wow! My working assumption is that finished buildings are very strong and hardly ever collapse. Why are under construction buildings weaker? Modern buildings are continuous structures, concrete poured down portion by portion into wooden forms around a welded steel wireframe. It's not like brick, steel and wood, where you build self-sufficient supports first and then lay down the flooring. Columns are themselves supported against collapse by the floors. Modern concrete can be very strong when cured, but the previous level is only starting to cure by the time you start building the next, so it isn't there yet either.
When it's complete, it becomes essentially just one solid piece of reinforced concrete.
You can see on the photos it's actually just wooden planks and plywood with still liquid concrete mix poured over it that gave.

Only a small percentage of weight is really lost in curing, it's commonly within 10%. Water reacts with cement chemically, it doesn't all just evaporate.
But it can take a long time to cure completely. Hoover Dam is only expected to reach its full strength by around 2050. Smaller buildings reach design strength in weeks to months.
