I was wondering what the worst play you've consistently seen is -- when you're sitting next to someone for a while and you watch them play a bit and they keep doing bad stuff, not simply one-off mistakes.

For me the worst happened a few days ago, playing DW44 and so was the guy next to me. If dealt one pair like 5589J he would hold the 55, but if dealt 2 pair like 55449, he would hold nothing and he did this several times. He would hold 2 to a royal (e.g. KQs) but not 3 to a royal, and I frequently saw him throw away 4 to a flush and 4 to an open-ended straight. The one time he did hold 4 to a flush he even messed that up because it was AJT9 all of hearts! He was playing the 25c denomination, relatively slowly (~500 HPH including some short breaks) and I saw him go through about $400 in 2 hours.

I don't know how consistent this guy is, but I just watched him hold A, 7, 2 offsuit on DDB UX while betting one credit per hand on five play.
