Depends on where you play and how the staff interprets the rules.

That sort of arbitrary and capricious interpretation is annoying to me. I would certainly question the Boxman for an immediate ruling. Quite frankly, I know some casinos have special deals on certain bets but that is a different matter than some arbitrary and capricious interpretation by a certain craps crew.

On this, the casino makes the call up to the full 5% vig, yet it is also best to avoid losing a customer, unless more trouble than he's worth.

Bottom line: Your mileage may vary. The dealer wasn't cheating. Just lazy.

Again, just avoiding a table skirmish.

Do the Dealers split the bet in two and lay them above the 4&10 boxes, or do they keep them together?

I think I know what you're suggesting. $100 given as $50 each on 4/10 are absolutely placed separately. But purchased together, they should charge $5 vig.

On a lay bet, vig is determined by how much the player stands to win. Since each would pay $25, each would have a $1 vig. Or total, they pay $50, for $2 vig.
