In your casino travels, have you seen a DigiDeal Classic Blackjack or Digital 21 table in the market with the Bad Beat side bet? If so, where (and how many)?
I've read one report here about it being in Indy Live. Anywhere else?

Basing those questions on the following :
There are four popular bonus side bets for the casino to choose from: High Tie Blackjack® Bonus and Upcard Luck 21™ Bonus, which are paid on the first two cards or “hole” cards; and the Bad Beat Blackjack® Bonus and Buster Blackjack™ Bonus, which are paid on the final hand outcome or “end game.” Now the operator can decide what type of bonus side bet and how many to offer its patrons: two, one or none at all. Multiple pay charts are available for each side bet. Maximum of two side bets in play at one time (one hole card and one end game).

I was the one who posted that I saw it at Indiana Live! It's the only place I have ever seen it. I've seen a lot of bonus bets, and that's the only place I've seen Bad Beat.
There are two racinos in the Indianapolis area - Indiana Grand (formerly Indiana Live!) in Shelbyville and Hoosier Park in Anderson. Both have the DigiDeal tables, and up until a couple of months ago, both had the Bad Beat side bet.
Indiana Grand recenly dropped the Bad Beat, and the dealers/table attendants have told me it was because the casino lost the rights to the software. As of two weeks ago, the Bad Beat was still on the tables at Hoosier Park.

What is your sense of whether the dealers at the Indiana Grand were upset that the casino lost the rights to the software? Did they still want to offer the game? (Because I can make that happen...)

There's no chance you've mixed up Hoosier Park and Indiana Grand, is there? The data I have says Indiana Grand still has six tables but Hoosier Park took out six tables at the end of March. Is that not the case?
Either way, you should ask the crew whether the bet is still actually available from the config menu. Unless the game's software has been updated recently, there's just a switch in an admin menu to toggle on/off the Bad Beat wager. Because the dealers' theory about "losing rights to the software" isn't correct. The e-table vendor still has rights to distribute the side bet, so there's no reason the casino can't offer it if they want to.
I was supicious that the story about losing rights to the software was a cover. I think the Bad Beat wasn't pulling in enough money - as evidenced by the fact that the players liked it so much. Next time I'm out there I'll ask whether it's still on the menu - right about the same time I suggest they turn down the "cheat" dial.

Seriously though, if the vendor's sales people aren't helpful enough, I can answer any questions they have. I'm pretty sure the game software has multiple paytables to choose from so the casino can set their house edge as desired.
