In the days of casinos being in the back rooms of upscale road houses located some distance away from city DAs and city politics, the participants were upscale types who drove their own cars, had a night out in evening dress, etc. Responsible gaming was possible because the clientele were by definition already responsible people.

I understand it was a bank error, but clearly this guy didn't have a "lapse in judgement" he took advantage of the situation and did so as often as he could. Think about taking out $1.5 million in cash in only 15 days, that is 15,000 $100 bills. He had to know it was an error. There is a big difference between a couple hundred bucks and millions of dollars. He knew the money wasn't his.

Its the sign of the times. Goverment believes that this is the easiest way to fleece the flock instead of raising taxes.
